Mandatory Enrollment Forms

All participants are required to complete the mandatory enrollment forms before joining the program. These forms are essential for registration, safety, and program participation.

Please ensure all sections are filled out accurately and a copy is saved or downloaded after submission, as it will be required for record and verification purposes.

Student Intake Form

Your student's basic information — full name, date of birth, school, grade, t-shirt size, and program-specific details. The first form to complete.

Dallas College Liability Form

Minor Student Consent, Photographic Release, and Liability Waiver. Essential for program participation on campus.

Authorized Pick-Up Form

Specify adults permitted to retrieve students from campus. Valid photo identification is required for student release.

Student Code of Conduct Agreement

Agreement on respect, engagement, and professional behavior. Signed by both the student and a parent or guardian.

Confirmation that you've read the Handbook and understand the policies. Finish this one last.

Log in to the Portal

The Program Portal is the main platform for all communication and updates throughout the program. Parents and students are required to log in regularly to stay informed about schedules, announcements, and important changes.

After completing and saving your enrollment forms, please log in to the portal. Once logged in, use the chat feature to submit all required documents and communicate directly with the program team for any questions or updates.
